Otsuka Station opened on 1 April 1903 as a station of the Nippon Railway's Yamanote Line. It handled freight from the start. On 1 November 1906 the line was nationalised under the railway nationalisation law.
On 1 October 1974 freight handling ended. On 1 November 1986 luggage handling ended. On 1 April 1987 the station became a station of JR East. On 20 October 1990 automatic ticket gates came into use at the north exit. On 15 December 1990 they came into use at the south exit.
On 18 November 2001 Suica became usable. On 18 March 2007 PASMO became usable. On 17 October 2009 a north-south passage opened. On 20 April 2013 platform doors came into use. On 12 September 2013 the shopping facility Atrevi Otsuka (アトレヴィ大塚) opened.
On 15 May 2017 the station was contracted out. On 31 October 2021 the Midori no Madoguchi closed. On 1 October 2025 JR East started its Eki-net Q Ticket service (えきねっとQチケ).
